Authorization to email utility bill - Village of Sherwood, WI

I hereby authorize the Village of Sherwood to email my monthly utility bill to me. I understand that by authorizing this, I will no longer receive a hard copy of my utility bill in the mail. I have a right to discontinue the emailing of my utility bill at any time by notifying the Utility Department. If you have any questions regarding your utility bill, please do not "reply" to the email you receive your bill from. Please call the Utility Department at 920-989-1589 or email at utilities@villageofsherwood.org .
